The two documents below are for the Kyushinkai Interclub MMA competition.
Please complete the Application Form and Medical Form and return to Sam Stewart or Scott Stewart at Kyushinkai Martial Arts.
Alternatively email the forms to info@kyushinkai.com, the forms will then need to be signed on the day.
Entries must be received by the 6th July 2012 with the £10 entry fee. Spectators can pay on the day.
